Cancellation and Refund Policy

1. Cancellation Policy Prior to Sample Collection: Clients may cancel their appointment or service before the sample collection process begins. To do so, please contact us at least 24 hours in advance. This allows us to manage our scheduling and offer the slot to another client. All Booked and Confirmed Appointments will incur a $75 fee if cancelled or No Show. After Sample Collection: Once the sample collection process has commenced, including mobile collection services or the receipt of a mail-in sample, cancellations will not be accepted, as the analysis process may have already started. 2. Refund Policy Services Rendered: We do not provide refunds for services once they have been rendered. This includes any testing or analysis that has been completed, regardless of the results. Before Services Rendered: Refunds for cancellations made before services are rendered, including sample collection, are considered on a case-by-case basis. Factors such as the timing of the cancellation and specific circumstances will be taken into account. Case-by-Case Basis: Clients requesting a refund before services have been rendered should contact our customer service team. Each request will be evaluated individually, and refunds will be issued at our discretion. Non-Refundable Fees: Certain fees, such as administrative or booking fees, may be non-refundable. These will be clearly communicated at the time of service purchase. 3. How to Request a Refund Contact Us: To request a refund, please contact our customer service team by phone or email. Provide your name, contact information, appointment details, and the reason for your cancellation or refund request. Please note that your refund request may be DENIED based on the circumstances for the refund. Evaluation: Our team will review your request and provide a response within 5 business days]. If approved, the refund will be processed using the original method of payment. 4. Exceptional Circumstances Emergency Situations: In cases of emergencies or unforeseen circumstances that prevent the client from proceeding with the scheduled service, we may consider exceptions to this policy. Documentation or proof of the situation may be required. Service Quality Issues: If you are dissatisfied with the service provided due to quality issues, please contact us immediately. We are committed to addressing and resolving any concerns you may have. Being dissatisfied DOES NOT guarantee you a refund.

Purpose and Scope: The reasons for conducting the test and the scope of the information it can provide. Procedure: An explanation of how the test will be conducted, including sample collection methods and any associated risks. Potential Results: Possible outcomes of the test, their meanings, and any limitations or uncertainties in the interpretation. Risks and Benefits: Information on the potential benefits of the test, as well as any risks or discomfort associated with the testing process. Confidentiality: Assurance that your personal and genetic information will be kept confidential and details on how your data will be stored, used, and protected. Voluntary Participation: Clarification that participation in the testing is voluntary and that you can withdraw consent at any time without penalty. 3. Process of Obtaining Consent Written Consent: For most genetic tests, we will require you to sign a written consent form that confirms your understanding and agreement to the testing.
